Question 1

Thinking about patients you have cared for in practice in year 1 as a student nurse, and drawing on the module content, can you explain to me how you would assess a patient?

• You can choose any appropriate tool or model that you have seen used in practice but will need to justify why you are using it.
  • Can you discuss what you would expect to find as part of the assessment  (including vital signs if appropriate)? Think of the questions you may need to ask to find out this information. How would you respond to the information that you find out?
  • REMEMBER you need to provide an evidence base throughout. As with academic assignments points that you make should be supported by appropriate up to date credible evidence.

Thinking about patients you have cared for in practice in year 1 as a student nurse, and drawing on the module content, can you discuss two interventions that would be used to change the health state of a patient? One of the interventions must be related to medicines management.

• Why did you choose these interventions? How would these interventions change the health state of a patient? How do they relate to your assessment?
  • What about the pathophysiology or pharmacology relating to these interventions.
  • For each intervention, consider the indications for using it , any contraindications, cautions or concerns.
  • Are there any side effects that you need to consider?
  • Do any other factors need to be considered- nursing considerations?
  • How will you monitor the effectiveness of your interventions?
  • Consider any relevant guidelines or recommendations- the evidence base of your interventions?

• What assessment methods / models/approaches / tools could be used here? Why?
  • Is there any supporting evidence? What information will these provide?
  • What ongoing care may be required – relate this to your chosen interventions
  • What potential members of the MDT may be needed to support an individual and why?
  • Is there anyone else who could be involved in a patients care?
