Based on the information you have put together in your two worksheets (Consumer Strengths and Limitations worksheet, and the Occupational worksheet), you can now write a case note describing the vocational goal you have chosen for your client. It is not enough to just list the name of the job – you must also justify why you believe that is an appropriate vocational goal based on your client’s skills, interests, abilities, and capabilities.
Your case note should be approximately one paragraph in length, single spaced, and include clear, concise, and direct statements that would convince your supervisor to approve of using agency resources to help your client achieve that goal. Resources are usually limited at any agency so you will need to demonstrate in your case note why you think that is the best job for your client.
